Abstract
Enhancement of a system s human reliability involves reducing the incidence of
human error in system operations. According to the International Maritime
Organization, human error accounts for 80% of accidents in maritime systems.
While the majority of these accidents involve ships at sea, there have been
several accidents involving ship collisions with port facilities. In 1996 the
Liberian bulk carrier BRIGHT FIELD crashed into the Riverwalk marketplace
at the Port of New Orleans. Property damage to the ship and to shoreside
structures was estimated at $20 million. The National Transportation Safety
Board (NTSB) cited several maintenance and crew communications errors as
